Comparative study between the values of ejection fraction obtained by means of 2D echocardiography and gated-SPECT.

Abstract

The decrease of the ejection fraction of the left ventricle is the best predictive factor of delayed mortality after a myocardial infarction. The ejection fraction of the left ventricle is a simple concept, it consists of the percentage of blood expelled by the left ventricle in each beat and values of 50% or superior are considered normal. In daily clinical practice it is the accepted value as parameter of the systolic function of the patient and appears like a trustworthy indicator of the future evolution of the patient with ischemic cardiopathy.
At present, varied techniques of image diagnosis offer us the possibility of obtaining the value of the ejection fraction of the left ventricle. The widely used is the bidimensional echocardiography (ECO-2D). The nuclear cardiology has had at its disposal, for years, diagnostic tools like the isotopic ventriculography that allows to obtain the ejection fraction of the left ventricle. In the 90’s the appearance of the cardiac tomogammagraphy (SPECT of myocardial perfussion) supposed a great technological advance for the precocious diagnosis of the ischemic cardiopathy. The myocardial SPECT provides information on the perfussion of the myocardial field in a noninvasive way. Later the gated-SPECT (GSPECT) appeared gated-SPECT. GSPECT studies are made simultaneously during the acquisition of the SPECT of myocardial perfussion and offer joint information on: perfussion, movement, thickening of the ventricular walls, allow the obtaining of parameters of global function like the ejection fraction of the left ventricle and ventricular volumes.
The GSPET uses software based on the geometric estimation of the volume of the ventricular cavity to determine the main hemodynamic parameters of the left ventricle. The use of compounds marked with 99mTc has meant important advantages from a technical point of view, achieving an increase in the sensitivity and the diagnostics or prognosis exactitude of these studies. Differences between the values of the ejection fraction of the left ventricle obtained by means of ECO-2D and GSPECT can exist.
