Age and comorbidities as factors of risk associated to the increase of the stay in the intensive care unit of cardiovascular surgery.

Abstract

The number of old age people is increasing progressively. In this group of population, the incidence of ischemic cardiopathy is very high, a greater number of revasculatization procedures are being made every day, among which the surgical treatment under extracorporeal circulation can be highlighted.
The aim was to know the relation between the age and the preoperating comorbidities, on the one hand, and the time stay in a intensive care unit (ICU), in the other.
From the total of discharges of the unit, all the patients undergoing cardiac surgery with a stay superior to 3 days were selected, during the three last years, 96–97-98. A descriptive, cross-sectional and retrospective study was designed in the patients who underwent a cardiac surgery.
They were divided in four age groups: <60, 60-69, 70-79 and >80 years. The data were divided in four categories: demographic characteristics, preoperating factors, operative variables, and postoperating complications.
The results were a growing increase as much in the stay in ICU, as in the proportion of patients >80 years. The recorded risk factors that influenced in the time of stay were: creatinina >2 mgrs/dl, fraction of ejection <50%, diabetes mellitus, hypertension (HTA), in addition to arrhythmias and the need of oxygen (O2).
We concluded stating that the increase of the patients’ age and the comorbidities increase the risk of postoperating complications and the time stay in ICU, therefore more nursing care will be needed.
