Abstract

Introduction: Rehabilitation programs are being applied more and more to a greater number of cardiac
pathologies. Although they need to be adapted to the characteristics of the illness and patient, there exists something in common amongst these patients; the difficulty to stick to the changes in lifestyle.
Objective: To evaluate the long-term maintenance of the changes to lifestyle and modifiable risk factors of patients included in our program.
Method: Patients with ischaemic cardiopathy, post valve change and post coronary aortic by-pass. Selected at hospital cardiology nursing. After three months they were discharged and referred to their GP at their local health centre. Repeat check-up after one year.
Results: 110 patients, average age 65, high blood pressure 89%, diabetic 26%, dyslipemics 81%, overweight 45%, smokers 29%. High blood pressure: after three months 23% after one year 24%, 78% followed diet. Diabetics: glucosylated haemoglobin 6.7 to 5.8, 79% followed diet. Dyslipemics: 81% down to 58%, 85% followed diet (same as before). Overweight: 45% to 38%, 49% followed diet, now up to 56%. 90% had given up smoking on admittance to hospital; at present 88% don’t smoke. Physical exercise went from 85% to 90%. 24% work. 60% retired. 82% have recovered their daily activities. 79% continue appointments at nursing.
Conclusions: The patients modified risk factors totally or partially, sticking to change. Discharge from
infirmary is an instrument to be potentialised. It works as a link between the patient and health care professionals.
