Continued education in specialized nurse consultation: the tool to maintain the changes in the lifestyle of the coronary patient.

Abstract

Objetives: To demonstrate the experimental efficiency of the continued education for the long term modification of habits of life in coronary patients (p), by means of nurse intervention on the factors of cardiovascular risk (FRCV) performed in the hospital nurse consultation.
Methods: Prospective study of intervention in cohortes on p including in clinical trial, that received education in programmed nurse consultation of +30 minutes of semester. The intervention was performed between august/96 and december/01. The preventive objectives were: Hypercholesterolemia (Cholesterol < 200 mg/dl), Hypertension (TA < 140/90), Smoke (total abandonment > 6 months), Diabetes (glucose < 120 mg/dl), Overweight (ideal body mass index [IMC] by age groups), Not exercise (aerobic exercise 20-30 minute/day), excessive alcohol drinker (BEA, abandonment).
Results: 120 were included p put under ACTP, with average age of 58.9+9.7 years and mainly men (88%). A 76% with smoke habit; 49% with overweight (IMC means = 27.6+2.9 kg/m2); 46% hypercholesterolemics (92.5% presented/displayed cholesterol > 200 mg/dl); hypertense 45% (TA > 140/90, 37%); 15% diabetics (61% basal glucose > 120 mg/dl) and 9.2% BEA (> 40g/alcohol/day). An average pursuit of 4.2+0.9 years of the 100% of the sample was completed. The average of FRCV by p diminished from 2.2+1.1 to 1.0+0.9 (p=0,001). The smokers happened of 47.5% of the total to 6.6%; the overweight from 49% to 46%; p with Cholesterol > 200 mg/dl from 38% to 21%; hypertense with TA>140/90 of 17% to 8%; and the BEA from 9.2% to 3,3%. In four years only increased slightly to the group of p with glucose > 120 mg/dl. In the final visit a 62% made daily aerobic exercise.
Conclusions: 1) A 78% of the audited patients modified total or partially their long term FRCV and a 57.5% assumed of safeheart lifestyles; 2) the best results were obtained in the abandonment of the
tobacco (only 6.6% had returned to smoke to the 4 years) and, in inversely proportional relation, worse in hiperglucemia and the overweight; 3) the continued education performed in the hospital in nursing in cardiology consultations was like a suitable and significantly effective resource, being a model to extend in the cardiovascular secondary prevention.
