Abstract

Transradial approach to cardiac catheterization is associated with fewer vascular complications. The management of these complications is poorly described in the literature. Nowadays, we need the knowledge regarding the development of these complications, their management and evolution.
Objetive: We propose a novel nursing flow-chart for management and treatment of the complications
associated with the transradial approach.
Method: from January 2001 to December 2003 a total of 3.907 cardiac catheterizations were done by radial approach. The nursing flow-chart was structured as follows: a) definition and classification of complications, b) signs and symptoms, c) conduct and follow-up.
Results: In 35 cases (0.89 %) severe complications were observed (haematomas > 6 cm, n = 13; fistulas, n = 2; perforations, n = 5; pseudoaneurysm, n = 1; radial occlusion, n=12 (6 with pulse and 6 without pulse); endarterectomy, n=1; sterile inflammation, n=1). All patients were treated conservatively with our nursing flow-chart and none need for blood transfusions or surgery.
Conclusions: This study suggest that management and treatment of transradial approach complications could be done with safe by nurse staff.
