Percutaneous antecubital vein approach compared to the internal jugular vein for coronary sinus cannulation in electrophysiologic studies.

Abstract

In many laboratories of electrophysiology, doctors use the internal jugular vein puncture as usual approach to cannulate the coronary sinus. This technique is very useful and sure in expert hands, however, carotid artery punction and pneumothorax production have been described as complications.
Antecubital vein cannulation by nurses can be an alternative to the internal jugular vein that would reduce the complications and the time of the electrophysiologic study, because it can be carried out simultaneously with other techniques carried out by doctor.
The objective of this study has been to analyze the benefits of the antecubital vein cannulation in comparison to the classic internal jugular vein catheterization for the coronary sinus approach in the electrofisioligic studies.
We have been studied in a prospective way 26 patients that were subjected to an electrophysiologic study and required coronary sinus cannulation. We carried out, in a random way, the percutaneous internal yugular vein approach by doctors as against antecubital vein by nurses and we compared the efficacy of both.
The following variables were analyzed: a) Veined Access. b) Sex. c) Age. d) Time used. e) Difficulty of the technique. f) Punction Success. g) Pain. h) Comfort. i) Coronary Sinus Success.
When we analyzed the results of the study, we could conclude that the antecubital vein cannulation to approach the coronary sinus in the electrophysiologic studies by nurses is a quick, simple and sure technique that provides smaller pain, bigger comfort to the patient and better fixation of the catheter in compared to the classic canalization of the internal jugular vein by doctors.
