Estudio comparativo de dos protocolos de control de glucemia en el postoperatorio de cirugía cardíaca.

Comparative study between two protocols of blood glucose control on postoperative patients in cardiac surgery.

Abstract

According to recent studies the strict control of glucose on blood, decrease the morbi-mortality in intensive care unit patients even cardiac surgery post-intervention patients.
It was performed a retrospective observational study over two patient population of 50 subjects undergoing to elective coronary arterial surgery excluding diabetics type 1.
First one we use the standard protocol (PS) since october to december of 2002 and the second one we use the new strict protocol (PE) since october to december of 2003.
It was compared the glycaemia average, the weight of nursery team work and direct complications (hyperglycaemia-hypoglycaemia) derived from his use.
The number of glycaemia in PE was 11.80±3.3 and 6.50±2.85 en PS (p<0.08). The glycaemia average (laboratory) in PE was 108.20±21.96 and 135.92±34.22 in PS (p<0.05). The insulin consume was great in PE than PS (0.017 u/Kg/h vs 0.006 u/Kg/h, p<0.05) and the number of interventions in the perfusion was five times more in PE than PS (p<0.05).
There was more hyperglycaemia in PS and there was no significant differences in the hypoglycaemia incidence.
According to this results we can see we have improve the glycaemia control over our patients, it had increase the nursery staff work burden and the level of complications was small.
